Participating Insurance Plans

Some of the insurance plans we participate with are:

• Michigan Medicaid

• Preferred One
• Wisconsin Medicaid

Because of the financial impact on you of whether or not a provider participates with your network, we suggest that you call the number on your insurance card or check the internet to verify provider status.  This will ensure that you get the most up to date information.

Billing Protocol

As a courtesy to our patients, we will submit all insurance claims.  Please keep us informed of the current address and insurance information.  Patients are responsible for payment of co-pays at the time of service.  If no insurance coverage is in effect, payment will need to be made in full at the time of service.
